1. Fill out our brief Hospital Information form and sign our Work Agreement (these are required only with your first submission).  To download these two forms, just click here. 
2. Have your patient fill out, sign and date the Authorization to Represent.  To download it, just click here. 
3. Print out your facility's patient face sheet and complete a financial assessment sheet, if applicable.
4. Position the above forms/sheets behind your fax cover page and your Business Associate Agreement, and then fax them all TOLL-FREE to QUICKReferral at (866) 352-2504 (or you can E-mail or download referrals if you prefer).
